The ByWard Family Health Team would like to inform patients that Dr. Quan-Yong Sim has made the difficult decision to permanently close his medical practice. 

The clinic is working to support affected patients through this transition. OHIP-insured patients who are currently rostered to Dr. Sim will be transferred to Dr. Jonathan Moulton, who recently joined the clinic and will assume responsibility for Dr. Sim’s practice effective July 1, 2026. 

Please note that Dr. Moulton provides medical services in English only and is not currently able to offer care in French. 

Patients who are insured through RAMQ or another provincial health plan outside of Ontario, as well as patients who are no longer rostered to Dr. Sim, will receive separate communication outlining next steps and available options. 

During the transition period, messages, test results, consultations, reports, and prescription renewal requests related to Dr. Sim’s patients will continue to be reviewed. 

Patients who require an appointment may contact the clinic directly. For urgent health concerns that cannot wait for a routine appointment, patients may also access the ByWard After-Hours Clinic. The After-Hours Clinic is intended to address urgent medical issues that require timely assessment but are not emergencies. 

We recognize that this change may be difficult for patients and appreciate your patience and understanding as we work through this transition. We thank Dr. Sim for his years of service and dedication to his patients and wish him all the best in the future.
