To begin, I would like to thank you for entrusting me with your health and wellbeing. It is with bittersweet regret that I will be retiring from my practice at ByWard FHT as of December 31, 2023.

At your next appointment – whether it’s in person or by phone – it’s best to discuss the options for your further care. For those not seen in consultation, the relevant documentation is available at your referring physician.

Note that your medical records will remain in the custody of ByWard FHT for a period of not less than 10 years.  For a copy of your medical records or to send it to another provider, ByWard FHT requires your written authorization.  An administrative fee for this uninsured service will apply.  A consent to release personal health information can be found on the ByWard FHT website at: https://bywardfht.ca/our-clinic/forms/ .
