I am writing this letter to inform you that I will be relocating my practice effective November 1, 2022, upon my return from maternity leave. I have thoroughly enjoyed my years of medical practice at Byward Family Health Team and have been blessed with an amazing group of patients and their families.

If I am actively following you or your child for medical care as a Pediatrician, and if you would like to join me at the new pediatric practice location, your Electronic Medical record can be transferred to Ottawa Medical Group in Orleans. There will be no disruption to your child’s continuity of care. If you have been discharged, or graduated from my practice, this letter is to inform you of my relocation and no further action is required. To continue in my new practice, please contact the clinic directly (Ottawa Medical Group) beginning October 2022 to book any future follow up appointments.
I do understand this location may be too far for some, therefore; I am happy to continue serving your child’s medical needs until you are able to find a new Pediatrician and ensure a smooth transition of care to the new doctor. There are currently two Pediatricians at Byward Family Health Clinic (Dr. Ronald Tam and Dr. Radmila Jovanovic) who are accepting new patients for pediatric consultations (non- primary care visits) by referral.
Please inform the Pediatric receptionist at Byward Family Health Team until October 31st, or my receptionist at Ottawa Medical Group in Orleans from November 1st, 2022, onwards, if you would like a referral created for one of the Pediatric Specialists at the Byward Family Health Team.
In addition, if you require a new Primary Care Physician for your child (for things such as well baby, child check ups, vaccines, etc.), you can contact Health Care Connect, a service provided by the Ministry of Health, to help you locate a new family physician at 1-800-445-1822.
If you would prefer to seek care elsewhere, that is not ByWard Family Health Team nor The Ottawa Medical Group, please be advised that your medical records will remain in the custody of ByWard Family Health Team for a period not less than 10 years. To send a copy of your medical records to another physician, via fax or mail, ByWard FHT requires your written authorization. An administrative fee for this uninsured service will apply. I would like to thank you and your family for the confidence that you have had over the years in my medical care. - What happens if I need to see a doctor soon?
Dr. Andrea Tam, my locum will continue to cover my patients at Byward Family Health Team, until October 31st, 2022. I will then see my patients in the new clinic at Ottawa Medical Group starting November 1st, 2022, until you transfer to a new Pediatrician, if you wish to do so. You may also visit a walk-in clinic or a clinic of your choice. If it is an emergency, you should visit your nearest hospital/ER as soon as possible. - What if I need my prescription renewed/refilled? Or results/reports to be reviewed?

Your new family doctor or Pediatrician can request the release of medical records with your signed consent, and we will send them to your new clinic. An administrative fee is applicable per Ontario Medical Association guidelines for clinical staff resources, including photocopying costs and postage. - What if I just want a copy of my records for myself?
You may request that as well. Put in your request with one of the receptionists (at Byward Family Health Team prior to November 1stor Ottawa Medical Group November 1st onwards). Depending on the size of the medical chart, and whether the medical data was transferred to the new clinic, the timeline may vary. An administrative fee is applicable per Ontario Medical Association guidelines for clinical staff resources, including photocopying costs and postage. - What if I want a copy of the records for my family/ minors?
For your underage children, you may request with ByWard Family Health Team or Ottawa Medical Group receptionists, in the same way as for yourself. For your other family members (husband, parents, or other children), there is written authorization/ consent form required, for adherence to privacy regulations. They can also directly request the records themselves and have it picked up by a family member.
