ByWard Clinic Management (BCM) operates a busy clinic and offers a comprehensive range of services to the general public. JOB SUMMARY 

We are currently looking for a Nurse Practitioner (NP) to join our team part-time (1.5 days) as a locum (independent contractor) and provide a full range of healthcare services to patients within the primary health care NP’s scope of practice. The NP will provide in-person care to patients of one family physician in support of her practice. The NP will have access to all the resources of our FHT (social worker, mental health counsellors, pharmacist, dietician, etc). Note that there is the possibility of working with other family physicians if so desired.

Typically, a locum is paid for each regular shift worked for the duration of this agreement. A regular shift is defined as a period within which the Locum sees 12 patients per shift, and can be scheduled between 9h00-12h00, or 13h00-16h30, or 17h00-20h00. Shift dates and times will be mutually agreed upon by the Locum and Signatory Physician(s). That said, there is flexibility in this arrangement. 

ACCOUNTABILITY

Staff are expected to work in a professional manner, demonstrating above average qualities such as courtesy, tact, and judgment with respect to patients, fellow colleagues, and professionals.  All staff are expected to work in parallel with the organization’s confidentiality agreement.  As a member of our team, you are responsible for bringing issues that affect the smooth operation of the organization, to the attention of your immediate supervisor.  It is your responsibility to be up to date with all organizational communication.

ESSENTIAL RESPONSIBILITIES

  • Initiate and manage the care of patients with diseases or disorders within the Nurse Practitioner’s scope of practice and/or monitors the ongoing therapy of patients by providing effective pharmacological, complementary or counseling interventions.
  • Evaluate effectiveness of care plans by assessing patient comprehension and compliance to the care plan, reviewing clinical, behavioral and learning outcomes.
  • Based on evidence and best practice and in networking collaborate in development of goals and objectives, services and activities, outcome measures and evaluation for student community.  By doing so, develops an information network for present & future use.
  • Integrate best practice guidelines and current evidence-based research into clinical practice,
  • Evaluate clinical outcomes and integrate nursing knowledge and research into clinical practice,
  • Other activities may be assigned in collaboration with the health services members that relate to a Nurse Practitioner scope of practice.
  • Develop linkages with external partners as appropriate to ensure continuity of care,
  • Work in a manner to ensure safety to oneself, patients and their families, colleagues and the work environment.

ESSENTIAL QUALIFICATIONS  

  • Current Registered Nurse Extended Class certificate and registration with the College of Nurses of Ontario
  • Graduate of a recognized Nurse Practitioner program
  • Current professional liability insurance
  • Minimum of two years clinical nursing practice experience as a Nurse Practitioner
  • Previous RN experience in hospital emergency department, primary care clinic or equivalent.
  • Bilingual preferred (French & English)
  • Excellent communication and interpersonal skills
  • Strong clinical and effective time management skills
  • Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team
  • Excellent health assessment skills, clinical judgment, analytical and problem-solving skills
  • Has completed an approved controlled substance prescription and management education program

JOB SUMMARY

Work with the physicians and other Allied Health Professionals of the BWFHT to treat patients in a variety of ways including the use of braces, casts, shields, orthotic devices, physical therapy or surgery. The primary focus of the Chiropodist will be to work to help patients eliminate foot pain, gain mobility and maintain healthy feet. The chiropodist is part of the clinic’s Diabetes chronic disease management program.

ESSENTIAL RESPONSIBILITIES

Assessment and Evaluation:

  • Complete health assessment through information gathering, lower extremity physical examination, patient health history and relevant clinical findings
  • Evaluation of overall lower extremity foot and ankle function relating to activities of daily living
  • Examination and review of lab tests, diagnostic tests and consulting medical and surgical notes
  • Assessment of the impact of an injury, disability or disease (rheumatoid arthritis/diabetes/sprains/strains) on foot function

Networking:

  • Educate and advise patients about the prevention and care of morbid conditions relating to chronic diseases like diabetes, HIV, etc.
  • Ensure continuity of patient care through exchange of information and communication with the patient, family members, and other service providers who ma be involved with the patient
  • Work as part of an interdisciplinary team, making referrals ad providing supports to patients of BWFHT
  • Build relationships with various community partners
  • Act as a resource for the team in assessing other social agencies and supports

Reporting:

  • Receive referrals from medical and other healthcare practitioners and consult with these referring practitioners to provide timely and optimal care for their patients
  • Maintain program statistics for purposes of evaluation and research
  • Complete and maintain all necessary documentation as per act and professional standards
  • Secure resources by preparing reports, competing application forms/letters as required

ESSENTIAL QUALIFICATIONS

  • University Degree or College Diploma of advanced Chiropody or equivalent
  • Member in good standing with College of Chiropodists of Ontario
  • Bilingual (French and English) an asset
  • A minimum of two years experience in a community and/or primary health care setting
  • Computer literacy: MS Word, Outlook, Excel, Presentation software, EMR and Internet

The Foot Care Nurse is responsible for providing quality foot care services to help patients eliminate foot pain, gain mobility and maintain healthy feet.  The Foot Care Nurse acts as a resource to clients and colleagues, can work independently and within a team, and is compliant with legislation and standards of practice. The Foot Care Nurse is a key member of the clinic’s Diabetes chronic disease management program.

  • Minimum of two years clinical practice experience as a Foot Care Nurse, including providing foot care services to people living with diabetes
  • Advanced and Diabetic Foot Care certificate is preferred
  • Current Level C CPR course or equivalent
  • Excellent interpersonal skills and a commitment to inter-professional care
  • Strong clinical and effective time management skills.  Experience in a high-volume setting would be an asset.

 JOB SUMMARY 

Under the supervision of the Clinic Manager, the primary role is to provide medical records support for the ByWard Family Health Team (BWFHT) physicians, allied health professionals (AHP’s) and administration.

ESSENTIAL RESPONSIBILITIES

  • Triaging and distribution of all incoming correspondence (mail and fax)
  • Reconciliation of documents relating to the medical record;
  • Prepare all documents for scanning ensuring staples and clips are removed and pages are oriented correctly and organized for efficiency;
  • Scan medical correspondence and allocate to appropriate patient records as per BWFHT naming conventions;
  • Ensure quality control i.e., correct patient name, date of encounter, document quality and imaging;
  • Ensuring all work is accurate in terms of format, spelling, and grammar allocated to the correct chart;
  • Photocopying, faxing, filing, and other routine administrative tasks;
  • Other clinic related duties as required by physicians, AHP’s and/or administration that are mutually agreed upon by the incumbent and direct supervisor.

ESSENTIAL QUALIFICATIONS 

  • Knowledge of clerical and administrative principles normally acquired through post-secondary education or equivalent experience
  • Computer literacy: MS Word, Outlook, Excel, and Internet
  • Strong attention to detail
  • Excellent communication skills, both verbal and written
  • Bilingual (French and English) an asset
  • Knowledge of medical terminology
  • Dependable, reliable, with the ability to work under pressure and meet deadlines
  • To work in a manner that is in compliance with staff and patient safety practices, policies and procedures of BWFHT
  • Good attendance

EDUCATION/EXPERIENCE  

  • Completion of secondary school or equivalent GED.
  • Completion of a college program or other specialized courses for medical administration required or a combination of training and experience.
  • Some experience in a medical environment
