ORGANIZATIONAL DESCRIPTION

ByWard Clinic Management (BCM), formerly the University of Ottawa Health Services (UOHS), offers a comprehensive range of services to the University of Ottawa students, faculty, staff, and the general public. BCM operates busy clinics open to University of Ottawa students, faculty and staff, other patients registered to a BCM family physician as well as members of the general public. We are committed to delivering timely, quality and collaborative assessment, treatment and health promotion, in a supportive and caring environment that is open and transparent to social, ethnic, and income factors.

JOB SUMMARY 

The nurse works in partnership with the family physicians and other members of the health care team to provide comprehensive primary health care to our patient population according to established standards and practices. The nurse focuses on providing assessment, screening, healthy lifestyle support, education and chronic disease management with a goal of improving health outcomes and facilitating access to services.

ESSENTIAL RESPONSIBILITIES

  • Triage and assess clients to identify appropriate nursing interventions.
  • Assess the immunization status of all patients and ensure immunizations are current.
  • Document accurate and pertinent patient information in a timely manner, including maintaining a comprehensive health summary or patient profile.
  • Provide and co-ordinate screening for health issues such as diabetes, hypertension, cholesterol, cancer screening, cardiovascular health, mental health, cognitive function and osteoporosis.
  • Utilize all health-care encounters as an opportunity to identify educational needs and provide health education (e.g., immunizations, screening, health lifestyle)
  • Monitor and maintain infection control practices according to established standards.
  • Advance primary care nursing through participation in professional development and practice development activities, mentoring students, orientating staff and participating in quality assurance initiatives.
  • Perform first-aid treatment on emergency cases; make the initial assessment and direct client to physician or nurse practitioner according to the degree of urgency.
  • Assist and/or perform medical procedures (e.g. ear irrigation, immunization, allergy shots, injections, IV therapy, dressing, suture removal, etc.) according to standards of practice.

 

ESSENTIAL QUALIFICATIONS

  • Registered Practical Nurse or Registered Nurse with the College of Nurses of Ontario
  • Current membership with the RNAO
  • Current First Aid and CPR certifications.
  • Bilingual (French and English)
  • Computer literacy: MS Word, Outlook, EMR and Internet.
  • The ability to plan, organize and prioritize, and foster a work environment conducive to the well-being of patients and staff.
  • Effective organizational, critical thinking, problem-solving and decision-making skills
  • Effective communication skills and ability to establish and maintain collaborative working relationships with team members and patients.
  • Minimum of one year of experience working in a family practice, community or primary care setting

Externally: Successful relationships with patients, representatives, and other clients that present themselves via telephone or in writing.  There are no supervisory responsibilities related to this role.

REPORTING STRUCTURE (To whom the incumbent reports to for different aspects of their job). The incumbent reports to the Nurse Team Lead.

ADDITIONAL INFORMATION:

  • The clinics are open between 8am-8pm Monday to Friday and from 9am-12pm on weekends. Must be available to work various shifts between those times.

JOB SUMMARY

Work with the physicians and other Allied Health Professionals of the BWFHT to treat patients in a variety of ways including the use of braces, casts, shields, orthotic devices, physical therapy or surgery. The primary focus of the Chiropodist will be to work to help patients eliminate foot pain, gain mobility and maintain healthy feet. The chiropodist is part of the clinic’s Diabetes chronic disease management program.

Essential Responsibilities

Assessment and Evaluation:

  • Complete health assessment through information gathering, lower extremity physical examination, patient health history and relevant clinical findings
  • Evaluation of overall lower extremity foot and ankle function relating to activities of daily living
  • Examination and review of lab tests, diagnostic tests and consulting medical and surgical notes
  • Assessment of the impact of an injury, disability or disease (rheumatoid arthritis/diabetes/sprains/strains) on foot function

Networking:

  • Educate and advise patients about the prevention and care of morbid conditions relating to chronic diseases like diabetes, HIV, etc.
  • Ensure continuity of patient care through exchange of information and communication with the patient, family members, and other service providers who ma be involved with the patient
  • Work as part of an interdisciplinary team, making referrals ad providing supports to patients of BWFHT
  • Build relationships with various community partners
  • Act as a resource for the team in assessing other social agencies and supports

Reporting:

  • Receive referrals from medical and other healthcare practitioners and consult with these referring practitioners to provide timely and optimal care for their patients
  • Maintain program statistics for purposes of evaluation and research
  • Complete and maintain all necessary documentation as per act and professional standards
  • Secure resources by preparing reports, competing application forms/letters as required

ESSENTIAL QUALIFICATIONS

  • University Degree or College Diploma of advanced Chiropody or equivalent
  • Member in good standing with College of Chiropodists of Ontario
  • Bilingual (French and English) an asset
  • A minimum of two years experience in a community and/or primary health care setting
  • Computer literacy: MS Word, Outlook, Excel, Presentation software, EMR and Internet

As a team member of the ByWard Family Health Team, you will:

  • Collaborate and/or consult as appropriate, with members of an interdisciplinary team within the clinic and externally with the community of care
  • Provide assessment of goals, linking with community partners, advocate for clients and manage social issues that are barriers to care
  • Provide culturally sensitive care
  • Plan programs of assistance for clients including referral to agencies that provide financial assistance, legal aid, housing, medical treatment and other services
  • Investigate cases of child abuse or neglect and take authorized protective action when necessary
  • Serve as members on interdisciplinary teams of professionals working with client groups
  • Provide mediation services and psychosocial assessments
  • Evaluate the effectiveness of counselling and social programs
  • Conduct research to keep up-to-date on the latest trends in social care work
  • Manage personal health and social information in accordance with privacy laws and ethical standards

ESSENTIAL QUALIFICATIONS/REQUIREMENTS

  • Bachelor’s degree or Diploma in Social Service Work
  • Registered and member of Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Vulnerable Sector Check (level 3) within the last six months
  • Bilingual preferred (French and English)
  • A minimum of 2-years experience providing services in a community or primary health care setting preferably
  • Exceptional knowledge and understanding of current, evidence based and best practice guidelines of social services system and resources
  • Well-developed communication and interpersonal skills in order to effectively engage community services and individuals, increasing awareness of services and establishes rapport with clients
  • Excellent communication and organizational skills
  • Ability to work effectively either in person or remotely (virtual appointments)
  • Ability to work in a computerized environment

JOB SUMMARY 

We are currently looking for a full-time Nurse Practitioner (NP) to join our team and provide a full range of healthcare services to patients within the primary health care NP’s scope of practice. The NP will provide care to patients within our FHT guidelines for health promotion, disease prevention and chronic disease management. The NP will work collaboratively with physicians and allied health professionals in our shared care model.

  • Initiate and manage the care of patients with diseases or disorders within the Nurse Practitioner’s scope of practice and/or monitors the ongoing therapy of patients by providing effective pharmacological, complementary or counseling interventions.
  • Evaluate effectiveness of care plans by assessing patient comprehension and compliance to the care plan, reviewing clinical, behavioral and learning outcomes.
  • Based on evidence and best practice and in networking collaborate in development of goals and objectives, services and activities, outcome measures and evaluation for student community.  By doing so, develops an information network for present & future use.
  • Integrate best practice guidelines and current evidence-based research into clinical practice,
  • Evaluate clinical outcomes and integrate nursing knowledge and research into clinical practice,
  • Other activities may be assigned in collaboration with the health services members that relate to a Nurse Practitioner scope of practice.
  • Develop linkages with external partners as appropriate to ensure continuity of care,
  • Work in a manner to ensure safety to oneself, patients and their families, colleagues and the work environment.

 

ESSENTIAL QUALIFICATIONS  

  • Current Registered Nurse Extended Class certificate and registration with the College of Nurses of Ontario
  • Graduate of a recognized Nurse Practitioner program
  • Current professional liability insurance
  • Minimum of two years clinical nursing practice experience as a Nurse Practitioner
  • Previous RN experience in hospital emergency department, primary care clinic or equivalent.
  • Bilingual preferred (French & English)
  • Excellent communication and interpersonal skills
  • Strong clinical and effective time management skills
  • Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team
