Cardiovascular
Cardiovascular disease (CVD) is the leading cause of death, disability, and hospitalization within Ontario and the Champlain region. 90,000 residents are living with CVD in the Champlain region. Although we know a lot about the risks for CVD and how to manage the disease, we don’t always make the best lifestyle choices.
What to Expect
- Patient is booked for a 1 hour initial assessment appointment where measurements (weight, height, BMI, WC) are taken, a cardiovascular risk factor profile is reviewed.
- The patient is educated on non-modifiable (age, gender, family history and ethnicity) and modifiable (blood pressure, cholesterol, diabetes, nutrition, physical activity, smoking, stress/anxiety/depression and weight management) risk factors
- A personal plan is developed with the patient to address their CVD factors
- The patient is referred to any community of FHT resources (Healthy Lifestyles, Craving Change, Smoking Cessation Program, Dietitian or pharmacist).
- Follow up is arranged as needed.
