Important Notice on behalf of Santé Alliance Health for OHIP patients of Dr. Anne Marie Allart, Dr. Louise Bourgault, Dr. Paula Tchen, Dr. Adnan Alrai, Dr. Meena Sharma and Dr. Milka Gostimirovic

We hope this notice finds you well in these challenging times.

This notice is to inform you that six physicians from the ByWard FHT have decided to relocate their practices to Santé Alliance Health, as of September 1, 2021.

These physicians include:

  • Dr. Annemarie Allart
  • Dr. Louise Bourgault
  • Dr. Meena Sharma
  • Dr. Adnan Alrai
  • Dr. Paula Tchen
  • Dr. Milka Gostimirovic

In addition, the following physicians will be joining the group: Dr. Stanislav Terziev, Dr. Justine Courtemanche, Dr. Yolanda Ma and Dr. Krishna Gopi Jeyakaran.

We anticipate that construction at our new facility will be completed in the Spring of 2022.  The permanent address of this new 5000 sq foot facility, designed for optimal patient care, will be located at:

  • Where will the clinic be located in the meantime?

    In the meantime (and as of September 1, 2021), Santé Alliance Health will be located at:

    1181 Hunt Club (suite 208) & 885 Meadowlands Drive East (suite 15)

  • What about my medical record?

    Be advised that as of September 1st, 2021 a copy of your electronic patient record will be automatically transferred to Santé Alliance Health.

    If seeking medical care elsewhere, your written authorization is required to release your medical record. Prior to September 1, 2021, contact ByWard FHT directly, via fax or mail, to send a copy of your medical record to another facility. An administrative fee for this uninsured service will apply. A consent to release personal health information can be found on the ByWard FHT website at: . If a request is made after September 1, 2021, patients are to go to the Santé Alliance Health website at

  • If I decide not to follow my current doctor to their new location, where can I go to find a new doctor?

    If you need to find a new family doctor, you can register with Healthcare Connect via their website or register via the College of Physicians and Surgeons of Ontario. On their website, you will find a link for “CPSO doctor search”. You can then find a doctor by selecting “advanced search” to narrow by region or “additional search” to narrow your query. Once this is done, you can call the doctor’s office to ask if they are accepting new patients

  • If I want to stay at ByWard FHT, will I be assigned a new doctor?

    Due to limited capacity, the ByWard FHT will not be able to accommodate patients who wish to register with another provider at the ByWard FHT, with the exception of uOttawa and uStPaul students and staff.

    Students and staff may contact the ByWard FHT at 613-564-3950 or go to the online registration form located on the ByWard FHT website in order to have their name placed on waiting list to be registered to a new primary care provider.If registering online, answer “yes” when asked if you are currently registered to a Primary Care Provider at ByWard FHT.

    While ByWard FHT is doing what they can to accommodate patients, adding your name to the waitlist does not guarantee a primary care provider. Walk-In services will remain an option for uOttawa and uStPaul students and staff only.

  • I don’t understand, I thought I was a patient of ByWard FHT?

    This is a common misconception. The clinic, ByWard FHT, is where your provider has their practice. The clinic’s role is to make available the amenities and the resources that your provider requires so that they are able to provide services to their patients within their practice. It’s important to note that the relationship is between the patient and their provider, not the clinic.

    Ideally there would be another provider that would want to open their practice at the clinic and would be willing to accept patients from another practice. The clinic’s responsibility is to facilitate the transfer but is not responsible in guaranteeing the patient a primary care provider.

  • The letter states that University students and staff can be put on a waiting list, I’m an alumni and a retired professor, can I be put on the waiting list?

    Unfortunately the clinic’s compliment of primary care providers, able to accept new patient into their practice, is very limited. As such we require whatever availability we do have to accommodate current and or new students and staff, per the clinic’s obligation to the University.

  • I have more question about this, who should I contact?
