Important Notice for Patients of Dr. Lisa Lezack
I know this last year has been very challenging and I hope you and your family are staying safe and healthy. It is with sadness I must inform you that effective May 1, 2021, I will be leaving my medical practice at the University of Ottawa Health Services (UOHS).
We are currently looking for another physician to take over my practice at UOHS. Patients will be informed as soon as a replacement is found. My present replacement will continue to care for all of my UOHS patients until April 30th, 2021. If after that date a new physician has not been found, you may see a physician at our Walk-in Clinic situated at 100 Marie-Curie. The service is open every weekday from 8 AM to 8 PM, and on Saturdays and Sundays from 10 AM to 2 PM (closed on Sundays from June to August). Ontario patients may also call the clinic to book an appointment with one of the nurse practitioners.
Until a replacement is found, please be assured that another qualified health practitioner will review all incoming labs, test results and follow up accordingly. If you are expecting results and have not heard within a reasonable time period, please contact UOHS to ensure nothing has been overlooked.
It has been a privilege to be your family physician these past many years. Thank you for the trust you have placed in me and for allowing me to be a part of your life.
