I know this last year has been very challenging and I hope you and your family are staying safe and healthy. It is with sadness I must inform you that effective August 1, 2021, I will be leaving my medical practice at the ByWard Family Health Team (BWFHT).
We are currently looking for another physician to take over my practice at ByWard FHT. Patients will be informed as soon as a replacement is found. If after July 31st, 2021, a new physician has not been found, you may see a physician at our Walk-in Clinic situated at 100 Marie-Curie. The service is open every weekday from 8 AM to 8 PM, and on Saturdays and Sundays from 10 AM to 2 PM (closed on Sundays from June to August). Ontario patients may also call the clinic to book an appointment with one of the nurse practitioners.
Until a replacement is found, please be assured that another qualified health practitioner will review all incoming labs, test results and follow up accordingly. If you are expecting results and have not heard within a reasonable time period, please contact ByWard FHT to ensure nothing has been overlooked.
If you choose to leave BWFHT, you should seek medical advice about any test results, imaging procedures or consultations ordered by any doctor seen after July 31st, 2021. For example, after any medical encounter, please follow up with your new family doctor or the ByWard FHT walk-in clinic for review of results and recommended actions.
If seeing medical care elsewhere, your medical records will remain in the custody of ByWard FHT for a period of not less than 10 years. To send a copy of your medical records to another physician, via fax or mail, ByWard FHT requires your written authorization. An administrative fee for this uninsured service will apply. and have a nurse find a doctor or a nurse practitioner for you.
2. Use the College of Physicians and Surgeons of Ontario’s “Find a Doctor” search:

b) Click on “Additional Search Options” to narrow your search
c) Contact the doctor to check if they are accepting new patients
