Important Notice for OHIP patients of Dr. Luisa Menjivar Ponce

Dear Patient,

I know this last year has been very challenging and I hope you and your family are staying safe and healthy. It is with sadness I must inform you that effective March 31st, 2022, I will be leaving my medical practice at the ByWard Family Health Team (FHT).

We are currently looking for another physician to take over my practice at ByWard FHT. Patients will be informed as soon as a replacement is found. Should you call the clinic to book an appointment from now until March 31st, our reception team has been instructed to book an appointment with either myself or one of my colleagues to ensure the quickest access to care as possible. If after March 31st, 2022, a new physician has not been found, you may book an appointment with one of the nurse practitioners.

Until a replacement is found, please be assured that another qualified health practitioner will review all incoming labs, test results and follow up accordingly. If you are expecting results and have not heard within a reasonable time period, please contact ByWard FHT to ensure nothing has been overlooked.

If you choose to leave Byward FHT, you should seek medical advice about any test results, imaging procedures or consultations ordered by any doctor seen after March 31st, 2022. For example, after any medical encounter, please follow up with your new family doctor for review of results and recommended actions.

If seeing medical care elsewhere, your medical records will remain in the custody of ByWard FHT for a period of not less than 10 years. To send a copy of your medical records to another physician, via fax or mail, ByWard FHT requires your written authorization. An administrative fee for this uninsured service will apply. A consent to release personal health information can be found on the ByWard FHT website at: https://bywardfht.ca/our-clinic/forms/ .

If searching for a new family doctor, these two resources may help:

  1. Register with Health Care Connect ontario.ca/healthcareconnect and have a nurse find a doctor or a nurse practitioner for you.
  2. Use the College of Physicians and Surgeons of Ontario’s “Find a Doctor” search: CPSO Doctor Search
  • Choose “Advanced Search” to find a doctor near you (by city/town or postal code)
  • Click on “Additional Search Options” to narrow your search
  • Contact the doctor to check if they are accepting new patients

  • What if I need to renew a prescription?

    Please be assured that another qualified health practitioner will respond to ongoing prescription requests addressed to your doctor.

  • What if I'm scheduled for an MRI in 4 months, who do I follow-up with regarding results?

    A qualified health practitioner will also review all incoming labs and test results ordered by your doctor and follow up accordingly.

  • Where can I go to find another doctor?

    If you need to find a new family doctor, you can register with Healthcare Connect via their website https://ontario.ca/healthcareconnect or register via the College of Physicians and Surgeons of Ontario. On their website, you will find a link for “CPSO doctor search”. You can then find a doctor by selecting “advanced search” to narrow by region or “additional search” to narrow your query. Once this is done, you can call the doctor’s office to ask if they are accepting new patients

  • What about my medical record?

    To send a copy of your medical records to another facility, via fax or mail, please contact ByWard FHT directly.

    Note that your written authorization is required to release your medical records and an administrative fee for this uninsured service will apply.
